Minimum Purchase Order Requirement: $75.00

F.O.B.: All prices are quoted F.O.B. shipping point from our Grand Rapids, Michigan Factory. Shipments are prepaid to destinations within the continental United States and are then added, as a separate line item, to the merchandise invoice.

Freight: Common Carrier truck shipments or air shipments are forwarded freight charges collect, unless otherwise agreed with Formax Mfg. Management.

Terms: Net 10 Days Payable in U.S. Funds. Visa, MasterCard, American Express, Discover Cards are welcome.

Past Due Invoices: New purchase orders in house will be held until all past due invoice payments are received. All past due invoices are subject to a 1-1/2% interest charge per month, not more than 18% annually.

Returns: All returns must be factory approved. Restocking charges may apply.

Our Warranty Policy: All products are guaranteed to be free of manufacturing defects and may be returned for credit, replacement or refund if not entirely satisfactory.
